IHSAA physical and concussion forms

Any student planning to participate in athletics must have a completed physical and signed concussion form on file at the school each year. Physical exams must be conducted no earlier than April 1 of the preceding school year.

Travel Permission

Each time the student-athlete will not ride the team bus to/from an athletic contest, this form needs to be completed, signed by parent, and turned in to the student-athlete's coach.
